Personal Fitness Training

Physical Therapy Services
Bee Well Collective is contracted with Medicare and BCBS IL PPO/ Blue Choice PPO and services will be covered at contracted rates. All other insurances will be Out of Network, payment will be submitted at time of service at cash rate listed below and we will provide you with a superbill to submit to your insurance on your own for reimbursement. Physical Therapy Evaluation

Appropriate for all new clients, a PT evaluation includes a full assessment, tests and measures, establishment of a Plan of Care (POC) and goals, and initial treatment. Length of evaluation 1-1.5 hours.
Physical Therapy Session

Appropriate once an evaluation has been completed, this session is personalized based on your POC and goals. Length of session 1 hour.

Includes an evaluation (1-1.5 hours) and 5 follow-up sessions (1 hour each) with a 12.5% discount for prepayment ($800 value). This is appropriate for clients with an expected Plan of Care of 6 weeks.
