Individualized Coaching Memberships

All coaching memberships include an initial 60 minute intake session and additional sessions that are 30-45 minutes in length. Sessions may be completed via telephone, Zoom or in-person. *After the 1st month 'try it out' pricing, a 5 month contract is required at the rate listed below.

Basic Coaching Membership

$75/month

Do you have some goals in mind but need an accountability and motivation partner to help commit to making some positive changes in your life? This package is best for anyone wanting to meet 1x/month. Membership includes 1 1:1 session/month, detailed follow-up email with goals, a weekly accountability text message, and a 10% discount on special events. Try it out 1st month for $65 with no commitment, then 5 month contract required at $75/month

Premium COaching Membership

$129/month

Are you ready to commit to your health and wellness? Change happens best with consistency and focus. This package is a more comprehensive membership that includes 2 1:1 sessions/month, detailed follow-up email with goals, a weekly accountability text message, a 15% discount on special events, as well as goal/habit tracking and coach-access between sessions via the Everfit App. Try it out 1st month for $99 with no commitment, then 5 month contract required at $129/month

Intro to Goal Setting & Accountability

$99/month

6 30 minute sessions over 3 months, this package offers an introduction and education on how to start holding yourself accountable and create SMART GOALS to make changes within your life. 3 month contract required to complete the training modules, will include homework to improve understanding of the process.

Individualized Wellness & Fitness Memberships

All Wellness & Fitness Memberships include an initial 75-90 minute intake session and additional sessions that are 60-75 minutes in length. Sessions may be completed in-clinic, at a meeting location of your choosing or via Zoom. *In order to see change, memberships require a minimum 6 month contract.

Essential Wellness & FItness Membership(2 sessions/month)

$180/month

All memberships will be individualized to meet YOUR goals and needs. Includes up to 2 1:1 sessions/month that will include all aspects of 'premium coaching' listed above plus fitness & wellness instruction, guidance, goal setting and 'training assignments' via Everfit and a 20% discount on special events. All sessions will be structured to meet you where you are at in your physical fitness and can include cardio, flexibility, balance and strength training.

Elite Wellness & Fitness Membership

$300/mo

All memberships will be individualized to meet YOUR goals and needs. Package includes up to 4 1:1 sessions/month that will include all aspects of 'premium coaching' listed above plus fitness & wellness instruction, guidance, goal setting and 'training assignments' via Everfit, plus a 50% discount on special events. All sessions will be structured to meet you where you are at in your physical fitness and can include cardio, flexibility, balance and strength training.

Bee Well Signature Memberships

The ultimate 'feel-good' wellness package, our signature memberships are designed to keep you feeling your best. Each session with be 1:1 with a doctor of physical therapy and will include whatever your body is needing that session- it can include assisted stretching, soft tissue massage, joint distraction, trigger point release, cupping, dry needling, electrical stimulation etc. Let us help you prioritize your well-being with a monthly membership.

Bee Well Basic Membership(1 session/month)

$99/month

All memberships will be individualized to meet YOUR needs. Includes 1 1:1 60-minute session/month.

Bee Well Plus Membership (2 sessions/month)

$175/mo

All memberships will be individualized to meet YOUR needs. Includes 2 1:1 60-minute sessions/month.

Physical Therapy Services

Bee Well Collective is contracted with Medicare and BCBS IL PPO/ Blue Choice PPO and services will be covered at contracted rates. All other insurances will be Out of Network, payment will be submitted at time of service at cash rate listed below and we will provide you with a superbill to submit to your insurance on your own for reimbursement.

Physical Therapy Evaluation

Appropriate for all new clients, a PT evaluation includes a full assessment, tests and measures, establishment of a Plan of Care (POC) and goals, and initial treatment. Length of evaluation 1-1.5 hours.

Physical Therapy Session

Appropriate once an evaluation has been completed, this session is personalized based on your POC and goals. Length of session 1 hour.

Physical Therapy Mini-Bundle

Includes an evaluation (1-1.5 hours) and 5 follow-up sessions (1 hour each) with a 12.5% discount for prepayment ($800 value). This is appropriate for clients with an expected Plan of Care of 6 weeks.

Physical Therapy Bundle

Includes an evaluation (1-1.5 hours) and 11 follow-up sessions (1 hour each) with a 20% discount for prepayment ($1550 value). This is appropriate for clients with an expected Plan of Care of 12 weeks (or 2 sessions/week).

Dry Needling Services

Dry Needling Services are a cash-based service only. Please note most conditions require an average 3-4 sessions for full pain relief/return to function.

Single Session 1-2 Muscles

Includes an intake evaluation and treatment of 1-2 muscles based on condition.

Single Session 3+ muscles

Includes an intake evaluation and treatment of 3+ muscles based on condition.

Comprehensive 3-session package

Includes an intake evaluation and 3 comprehensive dry needling treatment sessions based on condition.

Maintenance 6-session Package

Perfect for those who find benefit from dry needling and would like a maintenance plan to help manage their condition, this package includes intake if needed and 6 comprehensive dry needling sessions.

Personalized In-Home CareGiver Training

*Prices reflective of travel within 30 minutes of Lemont, Illinois. Travel fees can be discussed if located further.

Initial Consultation and Home Assessment

This initial visit will include an assessment of the home and discussion of personal goals related to caregiver training. We will set a plan including who will receive training and how many additional sessions are recommended. Consult will be 1 to 1.5 hours in length.

Follow-up sessions

These sessions will be personalized based on your goals and needs. These sessions can include up to 3 caregivers and can be completed in the home or public location of your choice (for community reintegration training). Session will be 1 hour in length.
